Work & Well fully coordinates state leaves with FMLA and ADA.  Our team will determine for which leaves the employee is eligible and send all the necessary information to the employee including for state leaves.   If paperwork is returned to us and we are not managing the leave, we will use the information for FMLA or ADA and forward it to the appropriate carrier or state agency.

Our goal is to make the leave process easy for employees by facilitating all eligible leaves.

• If managed by another carrier or a state agency;
  • Send out the required forms
  • Forward completed paperwork to HR or carrier
  • Use information for FMLA determination
  • Track time in Work & Well’s System
• If managed by Work & Well;
  • Collect all necessary information
  • Make determination and notify all parties
  • Track time
  • Conduct return to work follow-up
